The Thyroflex Symptom Survey is a legal document used for diagnosing and monitoring thyroid disease. It is necessary to fill every symptom listed on the survey with a score (0, 1, 2, 3). Please do not leave any blanks.
Make sure the Thyroflex Symptom Survey is completed by the patient on the day the Thyroflex is done. The Thyroflex Symptom Survey completed by the patient prior to being tested will not reflect the correct symptoms if completed 1-2 months prior to testing. Symptoms change all the time especially if the patient is on a protocol.
