CORE HORMONE CONSULTATION PROTOCOL
To obtain assessment for your patients, the following information must be provided to our licensed doctor and hormone specialist. The purpose is to establish a patient file and baseline information on the patient to comply with the medical prescription laws. Subsequent assessment for the same patient will only require the test results from the Thyroflex, unless additional medical information is requested by our Consulting Specialist for that patient’s specific needs.
